2008 Rivers Alive Event Registration Form

Site registrations are accepted starting June 5, 2008

Please fill out the following registration form to help Rivers Alive keep accurate records on our statewide cleanup events. This information will help us keep in touch with you so that we can provide you with updates on further resources available through Rivers Alive. These include T-shirts and patches for volunteers that register before July 31 (first come first serve basis), radio PSA (Public Service Announcement), educational posters, and promotional posters to help you advertise your event. In addition, your event may qualify for a 3 x 6 ft. Rivers Alive banner.

Information about your cleanup will be used to help you publicize your event through this website. Please consider providing us with a website address (URL) to direct people to with specific information about your cleanup event. We will be including the following fields on our cleanup web page: First and Last Name, Association, E-mail Address, Daytime and Evening Phone Numbers, Fax Number, and the site specific information.

T-Shirt Registration - Each year Rivers alive is able to secure funding to print and distribute a limited number of t-shirts to volunteers who participate in the program. All cleanup organizers who register by July 31 qualify for free Rivers Alive t-shirts, on a first come first serve basis. However, you must submit the following information. (please estimate the number of participants, if necessary)
